Instructions:
The last document that you would need to complete your deliverables is the IMM 5475 Form. By completing this form, you will authorize Citizenship and Immigration Canada (CIC) and Canada Border Services Agency (CBSA) to release information from your case file to someone other than yourself - in this case to Launch Academy.
This will allow Launch Academy’s contact officer to open communication with Launch regarding your immigration cases. We will be able to obtain information such as the status of your application and we will be able to change your address if you move.

NOTE: ALL APPLYING FOUNDERS WILL NEED TO SUBMIT A PERSONAL IMM5475 FORM. Please save the file with the founder name included.

Immigration Canada now requests a handwritten signature on your IMM Form. So please print the document and submit it with your handwritten signature on it. Once you have submitted your IMM Form 5475, save it along with your other documents.
