Instructions: As part of your deliverables, we need more information on which of your team members will be applying for Visas on the basis of the same commitment. In this spreadsheet, you will need to identify all members of the entrepreneurial team, who your essential* team members are, who your non-essential* members are, who will be applying for the work visa (optional), and who will be applying for their PR. In the column, role description please write detailed information about the role of each SUV participating member.
There is also an additional section for those team members bringing direct family dependents, please make sure to fill out the name of the team member & provide each of the family members’ information and their relationship to the team member.
Please ensure every section reflects accurately what appears on each member/family member’s passport and travel documents.
*Essential team members are those who will be applying for the Work Visa.
*Non-essential team members are those who will be applying for PR status only (no Work Permit application) with your Startup Visa application.
*If there are any other team members who hold shares within the company but will not be moving to Vancouver with your company, please also indicate this on your Member Chart as we will be required to provide this information to the IRCC.

Submission: Download the Detailed member chart template and fill out each section of the document with as many details as possible. Then, submit it by uploading your document below.
Once you have submitted your Detailed member chart, save it along with your other documents.
Also, for each member joining your team for our Maple program, if not done yet please do not forget to ask them to fill out our co-founders form.
*No need to fill it out yourself if you submitted your team’s Maple application form at the beginning of your application process.
